The most dominant bacteria in causing dental caries is Streptococcus mutans. The high prevalence of dental caries and the weakness of the strategies used indicate the urgent need for alternative medicine. Red Betel Plant (Piper crocatum Ruiz & Pav) belongs to the genus Piper. Red betel has been used empirically for various traditional treatments, one of which is the treatment of toothache. This study aims to obtain compounds and determine the chemical structure of secondary metabolites in Red Betel leaf which has the potential as an antibacterial of S. mutans that causes dental caries. The method used in this study consisted of four stages, the first stage was the preparation of simplicia and maceration using methanol as a solvent. The second stage is fractionation and purification by column chromatography (gradient method) and monitoring of compound components using TLC. The third stage is antibacterial testing of S. mutans bacteria using the disc diffusion method. The fourth stage is structural analysis using 1D-NMR and 2D-NMR. The results of the study obtained isolates of secondary metabolites with the characteristics of white crystals soluble in ethyl acetate. The results of compound characterization using 1D-NMR and 2D-NMR spectra showed that the compounds contained in the isolates were steroid compounds, namely stigmasterol. In the antibacterial activity test of isolates against S. mutans bacteria, it was found that stigmasterol compounds were able to inhibit the growth of S. mutans bacteria ranging from concentrations of 6%, 8% and 10%, based on the inhibitory activity of stigmasterol compounds classified as antibacterial agents with moderate activity against Streptococcus mutans.
